Website Privacy Policy and SMS Terms of Service

Alliance Wellness Solutions LLC (“Alliance Wellness Solutions,” “we,” “us,” or “our”) respects your privacy and is committed to protecting your personal information. This policy explains how we collect, use, store, and disclose information through our website, intake process, patient services, and communications, including text messaging.

This policy applies to individuals who visit our website, request information, schedule appointments, receive services, or communicate with us.

Protected health information is also governed by our HIPAA Notice of Privacy Practices, which explains our legal duties, permitted uses and disclosures, and your rights concerning medical records. This website policy supplements that notice; it does not replace it. You may request a copy by contacting our office.

We handle information according to applicable federal and Maryland law, including HIPAA and Maryland’s Confidentiality of Medical Records Act. Additional protections apply to certain mental health records and, where applicable, substance use disorder records protected by 42 CFR Part 2. Other applicable privacy laws may provide additional rights.

2. Personal Information We Collect

Depending on your interaction with us, we may collect:

  • Identifying and contact information: Name, date of birth, residential or mailing address, telephone number, email address, and information needed to verify identity.

  • Patient and health information: Reasons for seeking care, symptoms, medical and psychiatric history, medications, diagnoses, assessments, treatment plans, clinical records, and information relevant to your care.

  • Insurance and billing information: Insurance provider, member and group numbers, policyholder information, eligibility and benefits information, claims, payment history, and payment details necessary to process transactions.

  • Appointment and service information: Scheduling requests, appointment dates, attendance, provider information, and physical location when needed for telehealth services.

  • Representative information: Parent, guardian, emergency contact, or authorized representative details, when applicable.

  • Communication and consent information: Messages, communication preferences, signed forms, authorizations, SMS opt-in and opt-out records, and the date and method of consent.

  • Website information: IP address, browser and device information, pages visited, and technical logs generated through website use. Depending on the website’s configuration and your choices, cookies or similar technologies may collect additional usage information.

We collect information directly from you and, where authorized or otherwise permitted by law, from representatives, referring providers, insurers, and others involved in your care.

3. How We Use Information

We use information as appropriate to:

  • Respond to inquiries and process intake requests.

  • Verify identity, insurance coverage, and eligibility for services.

  • Schedule appointments and send reminders or administrative updates.

  • Evaluate health concerns, provide treatment, and coordinate care.

  • Process payments, submit claims, and resolve billing questions.

  • Maintain records, manage practice operations, and improve service quality.

  • Record and honor communication preferences, authorizations, and opt-out requests.

  • Protect systems, investigate security concerns, and meet legal obligations.

We limit collection and use to information appropriate for these purposes. Providing information through a website form does not automatically authorize disclosure of medical records or enrollment in promotional messaging.

4. How We Share Information

We do not sell or rent patient information.

Subject to applicable law and any required authorization, information may be disclosed to:

  • Healthcare providers and care partners for treatment and care coordination.

  • Insurers and payment processors for eligibility verification, claims, and payment.

  • Service providers working on our behalf, such as electronic health record, scheduling, communications, billing, hosting, and information technology providers. Their access is limited to authorized services and subject to appropriate confidentiality and security obligations. Business associate agreements are required when applicable.

  • Authorized representatives or other designated recipients, consistent with your permission and applicable law.

  • Public authorities or other legally authorized recipients when disclosure is required or permitted by law, such as certain reporting obligations, oversight activities, or valid legal proceedings.

A request for information does not automatically authorize its release. We evaluate the legal basis and any additional protections before disclosing sensitive records.

Psychotherapy notes and certain substance use disorder records receive additional protection. We obtain authorization or consent when required. Uses and disclosures that require your written authorization will not occur without it. You may revoke an authorization in writing, subject to actions already taken in reliance on it.

5. SMS Consent and Privacy

No mobile opt-in or text message consent will be shared with third parties or affiliates.

We do not sell, rent, transfer, or provide SMS consent or mobile subscriber information to other organizations for their marketing or promotional purposes. Your consent applies to communications from Alliance Wellness Solutions.

RingCentral and telecommunications providers process the information necessary to transmit messages and manage delivery and opt-out requests on our behalf. This operational processing does not authorize them to use your consent to enroll you in another organization’s messaging program or to conduct independent marketing.

6. SMS Terms of Service

If you consent to receive text messages from Alliance Wellness Solutions, you agree to receive conversational and informational messages, such as appointment reminders, scheduling updates, intake follow-ups, and responses to your service-related questions.

  • Consent: We obtain and record appropriate consent before sending messages requiring consent. Website SMS consent is optional, separate from other consents, and unchecked by default.

  • Choice: SMS consent is not a condition of receiving care or purchasing services. You may request another communication method.

  • Frequency: Messaging frequency may vary based on appointments and interactions with our office.

  • Charges: Message and data rates may apply.

  • Opt-out: Reply STOP to discontinue SMS messages. You may receive a confirmation of your opt-out.

  • Assistance: Reply HELP for support or call (667) 400-3679.

  • Promotional messages: Consent to appointment or service messages does not constitute consent to promotional texts. Any promotional messaging requires separate appropriate consent.

  • Delivery: Message delivery is subject to network availability and is not guaranteed.

Standard text messages may be visible to others with access to your device or account and may not provide the protections of a secure patient portal. Please avoid sending detailed health information by ordinary text message. Contact our office for a suitable way to communicate sensitive information.

Text messaging is not intended for emergencies and is not continuously monitored. For an emergency, call 911.

7. Storage, Security, and Retention

Information is maintained in electronic systems used for patient care and practice operations and, where applicable, in paper records.

We maintain administrative, technical, and physical safeguards appropriate to the information, including access restrictions, authentication controls, staff privacy practices, and secure handling and disposal procedures. Access is limited to authorized individuals with a legitimate need for the information.

No electronic transmission or storage system is completely secure. When a breach requires notification under applicable law, we provide the required notices.

We retain information for the periods required by applicable recordkeeping laws and for legitimate treatment, operational, and legal purposes. Retention depends on the record type and circumstances, including requirements for minors’ records. When retention is no longer required, information is securely destroyed or appropriately de-identified.

Opting out of text messages does not require deletion of medical records or consent records that must be retained.

8. Website Technologies and External Links

Our website and its service providers may use cookies and similar technologies to support functionality, security, and website performance. You can manage cookies through available website settings and your browser. Disabling some cookies may affect website functionality.

Any analytics or advertising technology must be evaluated for its handling of sensitive information and applicable consent requirements. Website use or acceptance of cookies does not constitute HIPAA authorization to disclose health information.

Links to external websites and platforms are provided for convenience. Their privacy practices are governed by their own policies.

Depending on the information and applicable law, you may have the right to:

  • Request restrictions on certain uses or disclosures.

  • Request confidential communications through an alternative method or location.

  • Revoke authorizations and change communication preferences.

  • Submit a privacy complaint without retaliation.

Where applicable consumer privacy law covers information outside HIPAA or other exemptions, additional rights may include access, correction, deletion, portability, and opting out of specified processing. These rights are subject to legal exceptions and retention requirements.

Access to a minor’s records depends on applicable law, the services involved, and the minor’s confidentiality rights. Our public website is intended for general information and is not designed for children under 12 to submit information independently.
